Advanced Care for Safe, Precise Cyst Removal Serving the Greater Boston area, Weston, Newton, and Needham

A cyst is a pocket of tissue that may contain fluid, other substances, or even air, depending on its location and type. These bumps are often caused by factors such as blocked glands, hormonal changes, bacteria, or dry skin cells trapped in pores. At Premier Dermatology, we provide expert diagnosis and surgical solutions for patients across Wellesley, Boston, Weston, Newton, and Needham.

What Is a Skin Cyst?

A skin cyst is typically a closed sac with a distinct “wall” or capsule. Distinguishing the specific type is key to effective treatment.

We commonly treat:

  • Epidermal Inclusion (Epidermoid) Cysts: This is what most people mean when they refer to a “cyst in the skin”. These cysts contain keratin and small, usually painless bumps below the skin. They are often harmless but can grow slowly over time.
  • Sebaceous Cysts: These develop when the sebaceous gland becomes blocked or damaged.
  • Pilar Cysts: Firmer cysts are often found on the scalp. This tends to run in families.
  • Cystic Acne: Many people—especially teens—experience large, painful, fluid-filled outbreaks that differ from standard cysts and require specific medical management.
  • Milia: These are small cysts usually 1-2mm in size that contain keratin and are typically found on the face.

Cyst vs. Other Lumps

Not every lump is a cyst. Accurate diagnosis is critical before any removal is attempted. A cyst usually feels like a smooth, round ball that rolls slightly under the skin.

It is frequently confused with a lipoma, which is a benign overgrowth of fat cells. The primary difference is the texture and depth; lipomas are often softer and deeper than cysts. During your consultation, we will determine the nature of the growth to plan the correct procedure.

When Should a Cyst Be Treated?

Many cysts are harmless and can be left alone. However, you should see a doctor immediately if your cyst:

  • Grows rapidly or changes significantly
  • Becomes infected, painful, or red
  • Occurs in a sensitive location
  • Makes you feel self-conscious due to its visibility
  • Causes irritation by rubbing against clothing or jewelry

Please avoid squeezing a cyst at home. Rupturing the sac internally triggers a massive inflammatory reaction and makes surgical removal much more difficult.

Treatment Options in Wellesley, MA

Premier Dermatology offers several options for cyst and lipoma removal, depending on the cyst type and its current state.

  1. Injections (Intralesional Therapy) – For inflamed, tender cysts or deep cystic acne, we often utilize intralesional therapy. This involves injecting a medication (typically cortisone) directly into the cyst to reduce inflammation. This method is designed to “dissolve” the bulk of the swelling quickly without the need for an incision or cutting.
  2. Incision and Drainage (I&D) – This procedure is best suited for acutely infected or painful cysts containing fluid. After numbing the area, we make a small opening to drain the contents, providing immediate pressure relief. While effective for pain, the cyst wall (sac) often remains behind because it is too fragile to remove during active infection.
  3. Surgical Excision – This is the definitive treatment for non-inflamed, stable cysts (such as Epidermoid or Pilar types). Under local anesthesia, we make a precise incision to remove the entire cyst sac intact. Removing the sac wall is the only way to prevent the cyst from refilling in the future.

Recovery & Aftercare

Recovery is generally straightforward, depending on the procedure performed. For surgical excisions, you may have a small bandage and sutures that remain in place for 7–14 days. We generally advise avoiding heavy exercise that stretches the wound area for a short period.

Conversely, for incision and drainage procedures, the wound may be left slightly open to allow drainage to complete, and you may need to apply warm compresses at home to aid healing.

Cost & Insurance Considerations

Coverage for cyst removal depends on the procedure’s nature. If a cyst removal is deemed medically necessary, insurance plans typically cover the evaluation and treatment. However, if a cyst is small, stable, and removed solely for appearance, insurance often classifies it as a cosmetic procedure. We will provide a transparent cost estimate before proceeding so you understand your financial options.

FAQ

Cyst Treament

Can I just use antibiotics to cure it?

Antibiotics alone rarely cure a cyst. While oral antibiotics can reduce surrounding redness and help calm an acute infection, they cannot penetrate the fibrous cyst wall to sterilize its contents. Once the antibiotics are stopped, the cyst often flares up again. Physical drainage or surgical removal is almost always required for a permanent solution.

Will I have a scar after removal?

Any surgical incision leaves a mark. However, a controlled surgical scar created by a dermatologist is typically a fine, straight line that fades significantly over time. This is far preferable to the jagged, depressed, or discolored scar that results from a cyst rupturing on its own. We also place incisions in natural skin tension lines to camouflage them whenever possible.

Does the procedure hurt?

We prioritize your comfort by using local anesthesia to completely numb the area before we begin. Most patients feel only a slight pinch from the numbing injection and no pain during the actual procedure. For inflamed cysts, we can also use numbing agents to make the drainage process as comfortable as possible.

Why does my cyst keep coming back after it was drained?

Recurrence is common if the “root” of the problem—the cyst sac—was not removed. Simple drainage (I&D) only removes the fluid inside, leaving the balloon-like structure behind to refill over time. This is why we recommend surgical excision once inflammation has subsided; removing the entire sac is the only way to permanently stop the cycle of refilling.

If I have a cyst on my scalp, will I lose hair during removal?

We understand that hair preservation is a top concern for pilar cyst removal. We utilize specialized surgical techniques designed to spare hair follicles. In most cases, we do not need to shave a large area; we can simply part the hair and make a small incision. Once healed, the fine scar is typically hidden completely by your surrounding hair.
